Overview
To start or restore water service with the City of St. louis Water Division the following steps and correct form(s) must be filled out below.
Applying For New Water Service
Please note that although this form is online for your convenience, it does need to be signed by the resident AND the owner, if different. Also, before water can be turned on, the Note of Responsibility (Page 3) needs to be filled out and signed.

Request to Restore Water Service
Instructions
- Fill out the appropriate form(s), listed above.
- Print and sign the form(s).
- Submit the forms to the Water Division.
