The City of St. Louis Health Department appreciates reports of problems with food establishments. Department personnel may already be aware of the problem and may already be working to resolve it, but sometimes your call is what's needed to learn about an issue.
Preparation
Provide as much information as possible. Include information on:
- Name of the establishment
- Address or intersection of the food establishment.
- Describe your experience.
- Include date and time if applicable and known.
