City of St. Louis Benefit Premium Only Plan (POP)

WHEREAS, the City of St. Louis maintains the City of St. Louis Premium Only Plan (POP); and

WHEREAS, the City of St. Louis desires to amend and restate the plan;

NOW, THEREFORE by virtue and in exercise of the amending power reserved to the City of St. Louis and pursuant to the authority delegated to the undersigned agent of the City of St. Louis, the City of St. Louis Premium Only Plan (the “Plan”) is hereby amended and restated in its entirety effective on August 9, 2022.
